病歴(びょうれき)clinical (case) history, medical history

カルテには病歴(びょうれき)が記載(きさい)されています。

Within clinical records one's medical history is recorded.

患者(かんじゃ)の現症(げんしょう)や経過(けいか)などを含(ふく)む記録(きろく)が病歴(びょうれき)である。

Records including the patient's current condition and their progress is referred to as their medical history.

病歴(びょうれき)が多(おお)すぎるせいで保険(ほけん)に入(はい)れなかったのだ。

I couldn't get insurance because my medical history was too extensive.

医師(いし)：「治療(ちりょう)に先立(さきだ)ちまして、病歴(びょうれき)をお教(おし)え願(ねが)えませんか？」
